Mod_Survey 3.0.10 (Stable) Mod_survey is an Apache mod_perl module which allows users to create their own Web questionnaires using an XML-based tag notation. It supports exporting of data into several file formats, including SPSS syntax, semi-colon delimited fields, and SQL script.
